Thank you for your interest in a Membership with V-LINC. Please fill out the following form as completely as possible to become a member of V-LINC.
This section of the application will ask you details about your needs, please be as specific as possible as this will assist us in serving your needs.

Once we have processed your application you will be hearing from us with any follow up questions or to schedule your consultation. Please allow up to 2 weeks. Your membership date will start from the date you begin using your membership services.
What Now?
If you have any supporting documents, such as an IEP, 504 plan, IFSP or other types of documents you may send them in to us or bring them with you for your consultation. This information will be beneficial to the Assistive Technology Specialist in coming up with reccomendations in support of your goals.
